VFIX Physical Therapy Clinic — Chiang Mai
Subjective Examination
Objective Examination
Cervical Flexion — reproduced tightness sensation at end range Cervical Extension — reproduced pain into the suboccipital area
Cervical Rotation — reproduced contralateral tightness
Cervical Lateral Flexion — reproduced contralateral tightness
Spurling Test: Positive
☑️ Cervical Flexion — WFL, no pain
☑️ Cervical Extension — WFL, no pain
☑️ Bilateral Cervical Rotation — WFL, no pain
☑️ Bilateral Cervical Lateral Flexion — WFL, no pain WFL = Within Functional Limits
FOLLOW-UP ประมาณ 1 เดือน
VFIX Physical Therapy Clinic 📍 Chiang Mai —
This case was previously presented after the patient completed 8 treatment sessions on August 20, 2026. Today, we would like to share the approximately 1-month follow-up update to illustrate the full progression — from the initial physical therapy assessment, through the course of treatment, to the patient’s real-life functional outcome in daily activities.
We would like to sincerely thank Mr. Thirask Tritepchanchai from Mae Sariang District, Mae Hong Son Province, for placing his trust in us and making the effort to stay in Chiang Mai in order to undergo continuous physical therapy assessment and care with VFIX Physical Therapy Clinic — Chiang Mai.
Before Starting Care | Subjective Examination
Male patient, 68 years old
Presented with pain, tightness, and fatigue in the neck and suboccipital region
Symptoms had been chronic for more than 7 years
Symptoms had become more troublesome during the 2–3 months prior to the initial assessment
Symptoms were more pronounced with prolonged sitting
Because chronic neck pain is not necessarily caused by muscular factors alone, the physical therapy evaluation included cervical movement assessment, symptom reproduction, and relevant special tests.
Initial Physical Therapy Examination | Objective Examination
Cervical Flexion — reproduced tightness sensation at end range
Cervical Extension — reproduced pain into the suboccipital area
Cervical Rotation — reproduced contralateral tightness
Cervical Lateral Flexion — reproduced contralateral tightness
Spurling Test: Positive
The examination findings were interpreted together with the patient’s history and symptom behavior in order to develop an individualized physical therapy plan of care.
After the 8th Treatment Session
📅 August 15, 2026
The patient reported that his overall symptoms had improved by more than 70% compared with before starting care.
Cervical movement findings on that date were as follows:
☑️ Cervical Flexion — WFL, no pain
☑️ Cervical Extension — WFL, no pain
☑️ Bilateral Cervical Rotation — WFL, no pain
☑️ Bilateral Cervical Lateral Flexion — WFL, no pain
WFL = Within Functional Limits
At VFIX, however, our evaluation process does not end simply because the patient feels better on the day of reassessment.
Another key question is:
What happens when the patient returns to real life?
Approximately 1-Month Follow-Up
At follow-up after completion of care, the patient reported that:
☑️ He was able to perform his daily activities normally
☑️ He no longer experienced neck or suboccipital pain at the time of follow-up
☑️ He was able to walk continuously for exercise for at least approximately 1 hour
☑️ He was able to drive from Mae Sariang District to Chiang Mai without his previous symptoms interfering during the trip
At this follow-up visit, reassessment of cervical range of motion and special tests revealed no abnormal findings.
From 7–8 Years of Chronic Neck Pain to Real-Life Functional Follow-Up
For us, the goal is not limited to asking, “Has the pain decreased today?”
We also need to understand:
How the neck moves
Whether the original symptoms can still be reproduced
How the patient tolerates sitting, walking, and driving
And, over time, to what extent the patient is able to return to normal daily function
This is why a detailed physical therapy assessment and meaningful follow-up are so important — especially for individuals with chronic neck pain, suboccipital pain, neck and shoulder discomfort, or long-standing symptoms.
At VFIX, we use the VFIX Technique & Protocol together with the Nerve De-Trapment™ Technique
Our plan of care is based on each patient’s examination findings and clinical response, rather than focusing only on the location of pain.
